Excused Absence Documentation Form

Team members — please allow THREE business days from the day you submit your request for a memo for delivery. Also, if your instructors prefer to receive one note with all your tournaments for the term and you know your travel schedule, submit all the tournaments together and make a note in the comments and I will send a concise note.

This form is for UF Speech & Debate Team members file with the Dial Center administrative office to have a memo sent to their instructors documenting travel for classes they may miss while participating on a scholastic team.

Students should also contact their instructors before the memos are requests and make arrangements for any missed classes and work.

  • Request for travel documention

    Please fill out the following information accurately. Your excused absence memo will be sent directly to your instructor through e-mail from the Dial Center office. Please remember to plan ahead. Letters may take up to three business days. **Contact your instructor and initiate a plan to make up work outside a memo of documentation. Instructor would always prefer to find out that you'll be traveling and missing class from you rather than the office.

  • Course and Instructor information

    Please will out the following information accurately. Your excused absence memo will be sent directly to your instructor through e-mail from the Dial Center office.
  • Enter the number of the course you will be missing. Ex.: SPC2608
  • Enter the class number (five digit number) of the course you will be missing.
  • Enter the full name of the instructor(s) of the class you will be missing. Make sure you let your instructor know before you contact the center that you'll be missing class and make arrangements to make up your work. Instructors get annoyed most often when they get an absence notice before they find out from a student about a tournament-related absence.
